This option is only available for life insurance policies that have a premium deposit account. A premium deposit withdrawal is a withdrawal of all or part of the value in the premium deposit fund/account. If a premium deposit account/fund exists on your policy, all of the value in the premium deposit account/fund must be withdrawn before a policy loan can be taken. Please refer to your policy or contact our Customer Service Center to see if your policy has a premium deposit account.
- If any of the items on the following list apply to your policy, please complete the Cancellation and Disbursement Request form (Non-New York) or (New York) paying special attention to the definitions and instructions on page 4.
- Collateral Assignment
- Trust Owned
- Corporate Owned
- Partnership
- Limited Partnership
- Limited Liability Partnership
- Sole Proprietorship
- Guardianship/Conservatorship
- Power of Attorney
- Requesting Premium Deposit Withdrawal amount of $50,000.00 or more
- If your policy is owned by an individual, you are requesting a premium deposit withdrawal of less than $50,000.00 and none of the above conditions apply, you may request the premium deposit withdrawal in any one of the following ways:
- Contact our Customer Service Center to request a premium deposit withdrawal by phone
- You may submit a signed written request. Please make sure your request indicates your desired income tax withholding and mailing instructions (if other than the address on record)
- You may submit the Cancellation and Disbursement Request form (Non-New York) or (New York)
Please use any of these options to send the Cancellation and Disbursement Request form or written requests.
